1. Call Body in Motion at 530-306-5511

2. Let us know who your doctor is so we can make sure they have a referral form for our services. 

3. You will need to contact your doctor to get the referral for Body in Motion Physical Therapy services. 

4. To confirm your insurance, we will need the information on your insurance card which we will get by phone. 

5. Once we have the above, you will need to print the intake forms off this site, fill them out and either scan and send them to heidilien@binmpt.com or have them available for your first visit. 

6. We will then set up an appointment for your first visit.
